The Wisconsin Veterans Home at King's four facilities have received a 5-star ranking for the first time, according to an October 2016 survey.
The facility was under scrutiny earlier this year for facility conditions and sub-par care, according to former and current residents and state senators who were contacted regarding concerns at the facility.
The Centers for Medicare & Medicaid Services Nursing Home Provider Rating Report is based on quality of care delivered, direct care staffing levels and annual healthcare inspection.
The Wisconsin Department of Veterans Affairs said only 15 percent of homes in the country receive 5-star ratings.
