A new report by the Wisconsin Department of Safety finds 4 code violations in connection with the Zippin Pippin accident on June 20th. The report does not show what caused two trains to collide sending 3 people to the hospital with injuries.

The two page report found that operator training records were not available at the time of the inspection, and one of the Zippin Pippin ride operators was not in the correct area when the accident happened on the ride back in June. The report ordered the City of Green Bay to provide appropriate training for staff to stay at their station and maintain the records properly.
The city must also repair a loose brake sensor, and determine why the two ride cars collided back in June.
"We're not proud of it, but these are minor violations that we'll fix," said Green Bay Mayor Jim Schmitt.
The ride cannot the reopen until all of the violations are corrected. The Mayor's office told NBC26 the Zippin Pippin will not reopen for at least a week.
The report did not explain how the accident occurred in June. That investigation is still ongoing, according to Mayor Schmitt.
Green Bay has sent the cars in question to Philadelphia for inspection. Mayor Schmitt's Chief of Staff, Celestine Jeffreys, tells NBC26 there are no new updates on the progress of that inspection.
